Summary
Giant lipomas are rare benign mesenchymal tumors that can cause discomfort and functional impairment due to their size and location. Although typically benign, their size and potential to mimic other soft tissue tumors pose diagnostic challenges. Imaging and molecular testing play a vital role in distinguishing giant lipomas from atypical lipomas, well-differentiated lipoma-like liposarcomas, and liposarcomas. This case report presents an 82-year-old obese female with progressively enlarging bilateral gluteal masses causing aesthetic discomfort and daily functional impairment. Pre-operative imaging via an MRI of the gluteal region confirmed the presence of two large benign lipomas. Postoperative histopathological analysis and molecular testing were conducted to exclude other types of soft tissue tumors. The report emphasizes the importance of a comprehensive differential diagnosis when evaluating giant lipomas. The decision to treat should be based on various factors, including tumor size, symptoms, and both functional and cosmetic concerns. Surgical excision remains the treatment of choice for complete removal, with early diagnosis and appropriate management essential for achieving optimal patient outcomes.
Key words
giant lipoma – gluteal mass – surgical excision – liposarcoma – soft tissue tumor
